Percutaneous transhepatic biliary drainage after failed endoscopic approach in patients with pancreatic cancer and situs inversus totalis.

چکیده
Situs inversus totalis is a rare entity with complete transposition of all viscera, making endoscopic, radiological, and surgical procedures challenging and complicated. Endoscopic retrograde cholangiopancreatography (ERCP) is the main treatment modality in patients with extrahepatic malignant biliary obstruction [1,2], but in patients with situs inversus totalis, this presents a technical challenge and often fails to achieve duct access [3]. In patients with extrahepatic malignant biliary obstruction, when ERCP fails, percutaneous transhepatic biliary drainage represents a good alternative to decompress the biliary system [4,5]. We describe two patients with situs inversus totalis and common bile duct (CBD) obstruction as a result of pancreatic cancer (●" Fig.1) in whom endoscopic retrograde cholangiopancreatography failed, but both were subsequently treated successfully with percutaneous transhepatic biliary drainage. It was decided to place the patients on the angiography table in the supine position but on the opposite side (feet first) without artificially reversing the image on the monitor. The procedure was performed after local anesthesia with a left subcostal access, between the X and XII ribs on the middle axillary line, using a 21G Chiba needle. The cholangiography showed a dilatation of the biliary tree with a stricture at the distal CBD (●" Fig.2a). A 0.18 guidewire was inserted into the bile duct, then the coaxial system was advanced into the bile duct and the intestinal lumen was reached with a 0.035 hydrophilic guide and a 4Fr MP catheter, and an 8-Fr internal–external drainage was positioned with an Amplatz guidewire (●" Fig.2b). Seven days after the procedure, a self-expanding metal stent (SEMS) (10mm in diameter, 68mm length) was placed at the level of the obstruction (●" Fig.2c). At the end of the procedure, an 8-Fr internal–external drainage was positioned and cholangiography performed. This showed a good patency and the correct positioning of the SEMS (●" Fig.2d). There were no complications in either patient during the procedures and the internal–external drainage has been removed from both patients.

Modified palliative biliary stenting in situs inversus totalis patient with carcinoma gallbladder: feasibility and technical details.
Knowledge of the anatomical variants is essential for all invasive hepatobiliary procedures such as endoscopy, surgery and radiologic interventions. Modification in standard therapeutic interventions may be required based on variant anatomy. We report a technical modification in a 75-year-old female with known situs inversus (SI) totalis with carcinoma gallbladder. Present case highlights the f...

CONTEXT Endoscopic transpapillary biliary drainage is the procedure of choice for biliary decompression in patients with unresectable pancreatic cancer. When ERCP is unsuccessful, the usual alternative is percutaneous transhepatic biliary drainage. Recently, the use of EUS-guided biliary drainage has been reported, but it is not clear whether it is feasible for this technique to find more wides...

CONTEXT Endoscopic retrograde cholangiopancreatography (ERCP) is the procedure of choice for biliary decompression in patients with unresectable pancreatic cancer. However, it may be unsuccessful in 3 to 10% of cases. When ERCP is unsuccessful, the usual alternatives are percutaneous transhepatic biliary drainage or surgery. Recently, several authors have reported the use of EUS-guided biliary ...
متن کاملHepatic Segrnentectomy on Primary Liver Cancer with Situs Inversus Totalis
We present the first case treated by hepatic segmentectomy in a 69-year-old woman with primary liver cancer and situs inversus totalis. The situs inversus did not cause any technical problems during the operation, which was conducted under guidance of intraoperative ultrasonography.
متن کاملSitus inversus totalis and secondary biliary cirrhosis: a case report
Situs inversus totalis is is a congenital anomaly associated with various visceral abnormalities, but there is no data about the relationship between secondary biliary cirrhosis and that condition. We here present a case of a 58 year-old female with situs inversus totalis who was admitted to our clinic with extrahepatic cholestasis. After excluding all potential causes of biliary cirrhosis, sec...
